Before making decisions about your end of life care, talk with your family and physician about your choices and concerns.  Click the appropriate form above for a Durable Power of Attorney/Living Will form for that may be used in the state of Florida.  If you live outside of Florida, check for the laws and forms that apply to your state or country.  You may find this information on Web sites for your state bar association or state healthcare administration office, or check with your family physician or attorney. The documents required for your state may be stored on your personalized "My CD."

After your forms are completed, enclose them with $5 cash or check in an envelope and mail it to us at the address below. Be sure to include your return mail address.
